PTO Reimbursement Form

Have you helped organize a PTO event and need reimbursement? You can find the form here.

All receipts must be attached to the form. Return the completed form and receipts to the PTO Treasurer via email (preferred) or the PTO Treasurer mailbox in the main office. The PTO Treasurer will send you an email when the reimbursement check has been written.
